Patients with obstructive sleep apnea have suppressed levels of soluble cytokine receptors involved in neurodegenerative disease, but normal levels with airways therapy
Abstract Purpose Obstructive sleep apnea (OSA) results in systemic intermittent hypoxia. By one model, hypoxic stress signaling in OSA patients alters the levels of inflammatory soluble cytokines TNF and IL6, damages the blood brain barrier, and activates microglial targeting of neuronal cell death to increase the risk of neurodegenerative disorders and other diseases. However, it is not yet clear if OSA significantly alters the levels...

Diagnostic value of laboratory parameters for the discrimination between erysipelas and limited cellulitis
Summary Background and objectives Erysipelas, caused by beta‐hemolytic streptococci, and limited cellulitis, frequently caused by Staphylococcus aureus or other bacteria, are skin and soft tissue infections characterized by typical clinical signs. However, despite the therapeutical relevance they are often not differentiated (e.g in clinical trials). Erysipelas are efficiently treated with penicillin, while limited cellulitis is treated with more wide‐spectrum antibiotics. This study investigates...

Moles: Intersexual and genetically doped
Female moles not only have ovarian, but also testicular tissue that produces male sex hormones - which lets them diverge from the categorization into two sexes. A team describes which genetic modifications contribute to this singular development.

Bone Marrow Reconversion With Reambulation: A Prospective Clinical Trial
Objectives Marrow adipose tissue may be modulated by physical activity and reambulation after inactivity. The aim of this study was to measure the effect of reambulation for up to 2 years after 60 days of bed rest on the lumbar bone marrow composition. Methods In a prospective clinical trial, 20 healthy men participated in a 60-day, 6-degree head-down tilt bed rest study. Serial 3-T magnetic resonance (MR) imaging measures of the lumbar spine were performed at baseline, after 57 days...

Classification of Mammographic Breast Microcalcifications Using a Deep Convolutional Neural Network: A BI-RADS–Based Approach
Purpose The goal of this retrospective cohort study was to investigate the potential of a deep convolutional neural network (dCNN) to accurately classify microcalcifications in mammograms with the aim of obtaining a standardized observer-independent microcalcification classification system based on the Breast Imaging Reporting and Data System (BI-RADS) catalog. Materials and Methods Over 56,000 images of 268 mammograms from 94 patients were labeled to 3 classes according to the BI-RADS...

The Cutoff Point and Diagnostic Accuracy of the Voice Handicap Index in Cantonese-Speaking Population
Voice Handicap Index (VHI) has been used extensively in research and clinical settings. It has been suggested to be used as a diagnostic tool in the literature. However, little is known if it could be applied as a diagnostic tool to the Cantonese-speaking population. The study aims to establish cutoff points and the diagnostic accuracy for VHI-30 and VHI-10 to discriminate between Cantonese-speaking dysphonic and nondysphonic individuals.

Cepstral Measures in the Fado Voice: Gender, Age and Phonatory Tasks
Cepstral measures are sensitive to gender, age and phonatory tasks. With a cepstral measure designated as the CPP, it was possible to confirm the vulnerability of the Fado singers’ voice. These were established at the vocal pathological threshold, which suggests a need for a direct clinical approach for these voice users.

Blood Glucose Estimation From Voice: First Review of Successes and Challenges
The possibility to estimate glucose value from voice would make a breakthrough in diabetes treatment: namely, remove the delay in the nonintrusive instantaneous blood glucose estimation, relieve medical budgets and significantly improve wellbeing of diabetics. In this review, different approaches have been described and systematized, in order to provide an objective snapshot of the state of the art. Since nonintrusive glucose estimation is notoriously difficult, we included a Transparence and Reproducibility...

Robot-assisted retroperitoneal lymphadenectomy in patient with type I papillary renal cancer recurrence after 5 years of follow-up
AbstractPapillary renal cell carcinoma (PRCC) is a rare cancer and is the second most frequent histologic type among all renal cell carcinoma, accounting for up to 15%. A 72-year-old man underwent a right radical nephrectomy 7 years ago with final histopathology diagnosis of type 1 PRCC with negative surgical margins. Five years after surgery, computed tomography scan imaging showed the presence of multiple masses suspicious for node recurrences disease localized in the renal lodge, in the inter-aorto-caval...

Laparoscopic resectional oesophago-gastroplasty: a novel technique for minimally invasive treatment of large high gastric lesser curve GIST involving gastroesophageal junction
AbstractA paramount factor in selecting the operative approach for gastric gastrointestinal stromal tumours (GIST) is tumour location. Tumours located high along the lesser curve of the stomach pose a challenge in laparoscopic resection. A 56-year-old lady presented with per rectal bleeding and loss of weight. Endoscopic and radiological investigations revealed a large gastric GIST located over the lesser curve with proximal margin <1 cm from the gastroesophageal junction (GEJ). We present the...

A rare and severe case of pronator teres syndrome
AbstractWe present the case of a patient with severe symptoms of proximal forearm median nerve neuropathy. Over the course of 5 years his condition progressed to encompass rare features of combined pronator teres syndrome (PTS) and anterior interosseous nerve syndrome (AINS). The aetiology was found to be pronator teres compression and was managed successfully by surgical decompression. Proximal forearm median nerve compression should be considered as a continuum with two classic endpoints. At one...

ACE inhibitor-induced small bowel angioedema, mimicking an acute abdomen
ABSTRACTAngiotensin-converting enzyme (ACE) inhibitors are the leading cause of drug-induced angioedema, being the face, tongue, lips and upper airway the most affected ones. We describe a case of a 32-year-old white female with angioedema of small intestine after 1 month of perindopril therapy. The patient presented severe abdominal pain, nausea and vomiting. Laboratory analyses revealed mild leukocytosis and abdominal computed tomography (CT) showed unspecific findings, including segmental jejunal...

Palatal steatocystoma simplex—a rare oral finding at an even rarer location
ABSTRACTSteatocystoma is a rare, benign cyst that mostly originates from a dermal sebaceous gland. It can be divided into steatocystoma multiplex—with multiple locations—and steatocystoma simplex occurring at a single site. The lesion is mostly located on the skin but can be found on other locations as well. This is the first case report of steatocystoma simplex that was found in the palate of a 37-year-old male. After resection with small safety margins and local wound dressing, no recurrence was...

Pedicled jejunal flap reconstruction for partial duodenectomy: a report of two cases
AbstractConventional jejunal reconstruction procedures for large duodenal defects include jejunal serosal patch repair and duodenojejunostomy and have some risks of postoperative complications. The pedicled jejunal flap is used for reconstruction following laryngopharyngectomy, esophagectomy and other gastrointestinal surgeries. We report two cases of successful closure of duodenal defects after partial duodenectomies by pedicled jejunal flap reconstruction. Case 1: A 72-year-old man was diagnosed...

The Sense of Effort: a Cost-Benefit Theory of the Phenomenology of Mental Effort
Abstract In the current paper, we articulate a theory to explain the phenomenology of mental effort. The theory provides a working definition of mental effort, explains in what sense mental effort is a limited resource, and specifies the factors that determine whether or not mental effort is experienced as aversive. The core of our theory is the conjecture that the sense of effort is the output of a cost-benefit analysis. This cost-benefit analysis employs heuristics to weigh the...
