Regional activation in the human longissimus thoracis pars lumborum muscle
Key points •Longissimus activity in the lumbar region was measured using indwelling electromyography to characterize the territory of its motor units. •The distribution of motor units in the longissimus pars lumborum muscle was mainly grouped into two distinct regions. •Regional activation of the longissimus pars lumborum was also observed during functional tasks involving trunk movements. •The regional activation of the longissimus pars lumborum muscle may play a role in segmental stabilization...

Greater post‐contraction hyperaemia below vs. above heart level: the role of active vasodilatation vs. passive mechanical distension of arterioles
Key points The immediate increase in skeletal muscle blood flow following contraction is greater when the contracting muscle is below vs. above heart level. This has been attributed to muscle pump‐mediated venous emptying and subsequent widening of the arterial to venous pressure gradient which can occur below but not above heart level. However, alternate explanations could include greater rapid onset vasodilatation and/or transmural pressure‐mediated mechanical distension of resistance vessels,...

Unusual Case Of Combined Aortic Root Replacement And Repair Of Hypertrophic Obstructive Cardiomyopathy
In this case, we describe an unusual case of dual pathology: i) hypertrophic obstructive cardiomyopathy (HOCM) and ii) aortic root aneurysm. We demonstrate the advantage of a single trans aortic approach to address all components of the HOCM disease. This is illustrated in a video highlighting the main steps of the procedure.

Surgical outcomes for anomalous left coronary artery from the pulmonary artery: impact of late presentation
Although surgical outcomes for anomalous left coronary artery from the pulmonary artery (ALCAPA) are excellent in the modern era with the coronary reimplantantion technique, mortality remains high in Chinese population. This study was undertaken to review the surgical management in our center and assess the midterm outcomes.

Lung Transplantation for Pulmonary Hypertension with Giant Pulmonary Artery Aneurysm
Aneurysm of the main pulmonary trunk (PAA) is a rare but severe complicating factor in patients suffering from pulmonary arterial hypertension (PAH). Many centers consider a PAA as an indication for a heart-lung transplantation. We herein aimed to summarize our institutional experience with a lung-only strategy in this complex group of patients.

In 2004, a prospective randomized trial demonstrated that, after 3 years, saphenous veins (SV) harvested with a No Touch (NT) technique had a greater patency than radial grafts for coronary bypass surgery. We now report the 8-year follow-up data of this trial.

Authors' Reply Re: “CT evaluation of occipitocervical joint violation by C1 lateral mass screw” by Jun-Song Yang, M.D; Hao Chen; Liang Yan, M.D; Peng Liu, M.D; Tuan-Jiang Liu, M.D; Da-Geng Huang, M.D; Ding-Jun Hao, M.D.
Thank you very much for your interest and inquiry regarding our study (Yeom JS, Buchowski JM, Park KW, et al. Lateral fluoroscopic guide to prevent occipitocervical and atlantoaxial joint violation during C1 lateral mass screw placement. Spine J. 2009;9(7):574-9.). As described in our study, we concur that the shape of the C1 lateral mass is somewhat triangular so that when evaluated on coronal reconstruction images the C1 lateral masses look like a bowtie (Fig. A). Regarding your concern that only...

Authors' Reply: Confounding and mediation to reveal the true association between systemic inflammation and musculoskeletal pain
We wholeheartedly agree with Klyne and Hodges [1] who emphasized in response to our Perspective [2] that the association between systemic inflammation and musculoskeletal pain is complex. In our Perspective, we called for a prudent interpretation of the literature because multiple confounders influence the association between low-grade systemic inflammation and musculoskeletal pain. A recent systematic review reiterated our viewpoint (“The potential role of confounding factors on the association...

Letter to the editor concerning “Multiple confounders influence the association between low-grade systemic inflammation and musculoskeletal pain. A call for a prudent interpretation of the literature” by Schipholt et al.
We read with interest the article by Schipholt et al. 2018 [1] which provides a timely perspective on controlling factors that influence the association between systemic inflammation and musculoskeletal pain. Although we agree with their point, it may not be as simple as it appears.

CT evaluation of occipitocervical joint violation by C1 lateral mass screw
We read with great interest the article by Yeom et al. [1]. The authors introduced the concept of the anterior arch target point to help prevent occipitocervical (C0–C1) and atlantoaxial joint violations during C1 lateral mass screw insertion. They defined joint violation as the contact of the screw with the inner margin of the subchondral bone using 2D and 3D computed tomography (CT) images. In the provided Figure (panel A), only the sagittal reconstruction view was provided. The height of the lateral...
